responsible for the delivery of pharmaceutical services at a level of
demonstrated proficiency to serve as a role model to other pharmacists.
Responsibilities:
Be able to approach and initiate a professional conversation with
physicians, nurses or other healthcare providers regarding a patient or
other appropriate situation.
Identify opportunities for pharmacy interventions which will improve
patient outcomes and standards of care for patients served by the pharmacy
department.
Promoting cost effective medication therapy alternatives to the medical staff.
Providing educational training to pharmacy, nursing and physician staff.
Provide targeted discharge medication teaching.
Requirements:
Florida Pharmacist License required
Clinical Pharmacist experience required
Residency in Infectious Diseases or Antibiotic Surveillance experience required

About our PHARMACY
The CHAS Pharmacy is located in one of the four CHAS clinics but is available
to all clinics via tele-conferencing equipment and the Automated Drug

Pharmacy Technicians are located in each of the clinics to aid with dispensing
and setting up of the tele-conference equipment for patient education by the Pharmacists.
